Essential Duties and Responsibilities:Provide an assessment of the client’s mental health and substance abuse needs to develop a service plan.
Provide supportive counseling and advocacy for clients suffering from co-occurring disorders, including substance abuse.
Provide DSM-5 diagnoses and the ability to utilize research-based treatment modalities effectively.
Understanding and practicing the professional ethics code of conduct and HIPAA compliance.
Maintain up-to-date client records and statistical data.
Prepare documentation for the client’s file (intake, encounter notes, treatment plan, etc.)
Report counseling/case management services to the supervisor every week.
Refer clients to a psychiatrist for appointments as needed.
Provide referrals for community resources and social services programs within Hamdard’s service tracks.
Attend staff meetings and other Hamdard Health Alliance activities.
Participate in staff training, development, and support activities.
Develop and implement new processes and procedures for services provided.
Travel to agency locations in Chicago, Northside, and Addison Township may be required
Other duties as assigned that will support Hamdard’s mission.
Qualifications:Master's degree in social work or counseling
Eligible to take LCSW/LCPC exam within 90 days of employment, a plus
Substance abuse experience is a plus but not required
Experience providing behavioral health services to adult and/or adolescent clients.
Experience with MAT-based services is a plus, but not required
High level of comfort working within the medical model on a team of primary care providers while using a consultative style.
Ability to exercise sound judgment in managing patient crises calmly, consistently, and equitably.
Ability to manage a large caseload of clients presenting with complex needs within a community health setting.
Bilingual a plus, but not a requirement
Current Illinois license as a behavioral health professional required (LSW or LPC)
Excellent organizational skills are required.
Compliance with organizational TB, influenza, MMR, HepB, Tdap, and Varicella vaccination policies is required.
